• Special items that are not stocked may be shipped in more than 5 business days

Procedures

1. Complete this order form and email to info@drmotivate.com
2. DO NOT PAY without receiving an official quotation or invoice.
3. The cut-off time for placing an order is 1 pm, which means order received after 1 pm will be handled in the next business day.
4. An official invoice will be issued and emailed as confirmation that the order has been received.
5. Arrange payment via the following payment methods:
   • Cheque
   • Bank deposit
   • FPS
6. Email the payment proof to info@drmotivate.com for verification.
7. The order will be shipped once payment is received.
